The following materials are required for application:
    - Mission and overview of all annual activities of the organization.
    - A statement of how these activities fulfill NSYL Standards for Membership and Accreditation.
    - Sample program materials from activities for the past year.
    - Most recent financial statement (990EZ of equivalent)
    - Listing of governing board, including board member affiliation.
    - Listing of key program volunteers and staff, including role and contact information.
